K-5 Student Handbook Acknowledgment
Electronic access to the Lake Hamilton K-5 Student Handbook and the K-12 District Student Policies are listed below and may also be found on our website (lhwolves.net) under the State Required Information page. Please complete this form for each child you have in grades K-5 to acknowledge receipt of handbook access (for example, if you have two students in grades K - 5, you will need to complete this form two times, one for each child).   If you would like to request a paper copy, please contact your child's school office.
